Coroner's Packet / Reports

For persons desiring public record information about a decedent, a "Coroner's Packet" will need to be requested of the Coroner's Bureau. The packet will include a deputy coroner's investigative report, an autopsy report (if one was performed) and a toxicology report, if done.

To request a copy of the "Coroner's Packet" please:

1. Send a letter that includes the following:
  • Decedent's name
  • Decedent's date of death
  • Coroner file number (if known)

2. Include the address where the reports are to be mailed.

3. The fee for the packet is $15.00 payable by check or money order.

4. Send your request to the following address:

Reports will be mailed as soon as they are ready.
